The sugar-free version contains the same 10-herb blend mix that includes sage, peppermint and linden flowers that grow in the Swiss mountains.

The main difference appear to be the amount of menthol in each cough drop.

There’s only 1.1 mg of menthol in the sugar-free version while the original Ricola contains 4.8 mg menthol in each cough drop.
